Our recertification interviews are now Telephone Interviews. You do not need to come into the office to see your worker, they will call you at the scheduled date and time, and at the telephone number listed on your Appointment letter. If you have recently updated your telephone number, please contact us at (440) 284-4300 to advise us of your new number as soon as possible. We will be unable to call you for your interview if you have a long distance number or have reported no phone. In these instances, please advise us of a local number to reach you at, or you will have to come to the office for a face-to-face interview.
A recertitication packet will be mailed to you approximately two weeks prior to your scheduled telephone interview. Please complete all of the necessary forms, sign and date where noted, and return to the agency prior to your scheduled interview date. You may return the recertification packets via mail or you may bring to the agency front desk. This will assist us in getting your case authorized timely. If additional verifications are required, your worker will advise you.
