Adult Protective Services

LCDJFS is mandated by state law to receive and investigate reports of abuse, neglect, and exploitation of impaired individuals age 60 or older.  Social service workers assess all such complaints, and if necessary, arrange or provide a wide variety of services to reduce or eliminate the endangerment of at-risk adults.  Emergencies are investigated within 24 hours, and all other reports within three working days. 

Anyone wishing to make a report should call the Adult Protective Services direct line at (440) 284-4465 (if the call is unanswered, please leave a message on the voice mail), or complete this form and forward to our agency, either via mail, fax to (440) 323-3422, or drop off at our front desk.   Referrals are confidential and come from many sources including hospitals, home health agencies, family, friends, and neighbors.  Reports may be made anonymously.  In cases of after-hours emergencies, police, fire, and hospitals have a pager number to reach us.
